Pre-Departure Testing Requirements for Non-Chinese Citizens

In order to reduce cross-border transmission of COVID-19, you are kindly required to read and follow the pre-departure testing requirements which apply to non-Chinese citizens from 17 January 2022.

Non-Chinese citizens submit all following documents to and apply for the health code viahttps://hrhk.cs.mfa.gov.cn/H5/

1. Passport photo page

l If you are not an Australian citizen, please upload your Australian visaas well.

l Valid Chinese visa(must be issued after 27 March 2020)

l Valid Residence Permit for Foreigner in China

(only three purposes are valid:work, reunion and private issues)

l Valid Foreign Permanent Resident ID Cardof China

3. Standard test report

(must print name, date of birth, passport number,collection date, test site and negative result)

4. Vaccination certificate

(must printtravelers name, vaccine brand name, dates of all received doses)

5. Itinerary of a direct flight from Melourne to a Chinese city

(must print name and flight information)

6.Statement of Commitment on Personal Health Monitoring (Attachment 2)(To be examined by airlines before check-in.)

Passengers with infection history

Infection history are defined as any of the following COVID-19 results ever returned positive

l PCR

l IgM (Spike)

l IgM (Nucleocapsid)

l IgG (Nucleocapsid)

Passengers with infection history have to go through Step

Initial Review

Send following documents by email: hsjcmelbourne@gmail.com

Email Subject: "Initial Review: Name + Mobile Phone Number

l CT or X-ray (lung) clearance (the pregnant are exempted with medical certificate)

l Two PCR tests (24 hours apart)

 Quarantine

l Quarantine for 14 days once initial review approved

l Fill in the Personal Health Monitoring Form (Attachment 1)

 Pre-departure testing as per your vaccination status

Close Contacts or People with Suspected Symptoms

l Quarantine for 14 days

l Fill in the Personal Health Monitoring Form (Attachment 1)

l AdditionalPCR tests onDay 1, 4, 7 during the quarantine.(negative required)

l Pre-departure testing as per your vaccination status

Important Reminders

l Chinese Consulate in Melbourne only issues health code for passengers on a direct flight departing Melbourne for China.

l Take all protective measures before and during the trip. Reduce social contact and avoid dining out, shopping and gathering before departure.

l All your test results may be verified by airlines before check-in. Bring them with you.

l Don’t forget the Statement of Commitment on Personal Health Monitoring (Attachment 2)

l Airlines may have other conditions before the check-in, which may include a rapid COVID-19 testing and the temperature checking. Please check with the airline company in advacne.
